What Is a Patient Presentation?

A patient presentation is the structured verbal summary a medical student or resident delivers to their attending physician about a patient's clinical status. It typically covers the chief complaint, history of present illness (HPI), relevant past medical history, physical exam findings, key lab and imaging results, and a closing assessment with a plan. Mastering the patient presentation is one of the most important clinical skills you'll develop in medical school — it's how attendings evaluate your reasoning, communication, and readiness to care for patients independently.

Frequently asked questions

How do you present a patient on rounds? Start with a one-liner: the patient's age, sex, and chief complaint. Then walk through the HPI in chronological order, followed by relevant PMH, medications, allergies, physical exam findings, pertinent labs and imaging, your assessment, and your plan. Keep it concise — most attendings expect a presentation under 3 minutes unless told otherwise.

What is the SOAP note format? SOAP stands for Subjective, Objective, Assessment, and Plan. The Subjective section captures what the patient reports (symptoms, history). The Objective section includes vital signs, physical exam findings, and lab/imaging data. The Assessment is your clinical impression or diagnosis. The Plan outlines the next steps for workup, treatment, or follow-up. SOAP notes are the standard documentation format used across most clinical rotations.

How long should a patient presentation be? On most rotations, a new patient presentation should take 2–4 minutes. Follow-up presentations (daily progress notes) are typically much shorter — 1–2 minutes — focusing on overnight events, updated vitals, and any changes to the plan. Always follow your team's preferences, as attending expectations vary by specialty and institution.

What are the most common patient presentation mistakes medical students make? The most common mistakes include reading directly from notes instead of presenting from memory, including irrelevant details that obscure the clinical picture, skipping the one-liner, presenting the assessment without defending your reasoning, and neglecting to have a plan ready. Being overprepared on your plan — even if it's wrong — shows initiative and earns respect from attendings.
